"Well-intentioned". If well-intentioned ends in tragedy is it not as bad as one that was purposefully disastrous?

How does a disastrous end become less so because it was "accidental"? Isn't OUTCOME all that matters always and forever? "I meant well"! Big deal. So what? Who cares? You caused mayhem and tragedy due to your stupidity. Anyway how do we know that you "meant well"? Talk is cheap. Results count. The rest is merely windowdressing packaging.
