The patient who should have had the surgery had a blood clot on the brain. A few hours into the surgery the surgeons realized they were operating on the wrong patients because there was no blood clot. That patient is recovering and in no danger. Whose fault? Apparently the tag on the patient was wrong. Was it the tagger's fault or the fault of the doctors for not verifying?
