An ENT surgeon? Although I can't imagine any ethical surgeon would consider performing such a dangerous mutilation to someone.
I'm seriously hoping that is a "Photoshopped" image though. If that bit of jewelry were to become dislodged or the uvula were to elongate from inflammation the person could actually choke to death. (I experienced that problem, Mine would dramatically elongate while I was sleeping, becoming about the size of my thumb. I would wake up choking on it, had to have an ENT surgeon remove it. Not fun.)
