    Bad charcoal. It was two years old and wouldn't light using a chimney. It probably absorbed moisture from the air.

    I never had a mishap until I bought a gas grill.  I burned everything on it.  After using it several times I pulled the innards out of it, put a cookie sheet on the bottom covered with foil, and added charcoal.  I used it like that for years and never burned another thing.
