    We actually do see this in a number of domains where "efficiency" (very loosely defined) becomes such a target of fixation that other goals or considerations get lost. For example, the recent debacle with Volkswagen and its diesel cars. The engineers were so fixated with meeting fuel-consumption (efficiency) targets that they flim-flammed their way through complying with emissions regulations. That focus on "efficiency" ended up costing VW something like $18 billion and a huge hit to its reputation.
