Well, in controlled production, the producer has a "lot tolerance percent defective" that he is willing to accept in his product.
As Walt O'Reagun suggests, random reproduction with the associated DNA coupling can have some problems.
Although I did read some years ago that the human race has a built in system that somehow produces spontaneous abortions when the embryo has certain defects.
On a one-off production you can depend on people doing their very best in all phases of the production. With mass-production the work quickly becomes routine and the work tend to be more sloppy. Cars come off the assembly line with not all bolts tightened, etc.
