     The brain controls everything including when to activate pain.  

    The brain does not say OH INSANE.  NO PAIN.  

    Insane people may be in pain 24/7.  Some are some are not.  It is a separate condition.

    Yes, insane feel pain. Some maybe kept doped up so much that they are numb to some of it.     But it a pretty basic nerve response.
