    If you do not know the cause one cannot fix anything. My favorite phrase is

    I do not want excuses I want solutions

    I am confused Archerchef. My fault I'm sure, not yours. But if you cannot fix anything without knowing the cause how does that relate to "excuses"? Aren't they different things entirely? Don't you WANT TO KNOW the source/cause/catalyst/defect so you can address that and in so doing find a solution/solve the problem? Assessing blame is not the same thing as excusing or justifying or rationalizing. Clearly what you said and how I understand it is faulty. What am I not understanding?     Most people in situations will give you excuses as to why something went wrong.  To find a solution ones needs to correctly identify the cause, then resolve the situation if possible.

    Think of CYA situations and the miscreant giving you BS to cover their A__. 

    I am a firm believer in finding the root cause to most situations.  I am a fair albeit hard manager/director in these situations.  Several junior folks found that out a couple of weeks back and now their desks are empty.  They made excuses and did not find solutions.
