Is it his fault or society's? What's wrong?

Each relationship failure has its own reasons.

Commonly poor communication and bad actions of one or both parties.

It is subjective.  I know men who will never be in a successful and happy relationship, not because the woman doesn't understand him or want to make him happy, but because in his mind he is perfect.    I'll fall back on Freud on this and say "It's his mother's fault."

There are also some people (men and women) who are simply unable to form relationships (no matter how much they try).  It's not their parent's fault, rather it's a psychological disorder.

Inability to form relationships is less common than being able to sustain a happy, peaceful, and content state of being with another human.

I think part of the problem (for both men and women) is that they expect the other person to make them happy.  If you aren't happy with yourlself, no one is going to make you happy.
