It's just an expression. You're not really fighting the fire, you're fighting the person who's throwing the fire at you. Sometimes responding in kind is best.
People sometimes want their enemy taking a defensive position. They want their enemy reacting to them, instead of bringing the fight to them.
No, I don't fight defensively. If someone messes with me I turn the other cheek or I fight back aggressively.
