    I always ask for a few additional napkins.  I like to get a fabric napkin and create for myself a placemat if the table is not clothed.

    As many as are provided, as I tend to eat in establishments that give you the cutlery for each course wrapped in its own paper serviette.

    I usually also am reading a book and three it seems to be.
    I admit to maybe more if I ordered onion rings or fries; I don't often eat those with utensils.
