Like BP in the Gulf or the Exxon Valdez off the Alaskan coast? In both cases environmental disasters could easily have been averted had corners not been cut in the pursuit of cost minimising. I'll go one further than that, the Daiichi meltdown at Fukushima needn't have happened either, had the seawall been built to the originally recommended height (ten feet higher than it actually was). Even FIVE feet higher would have kept the tsunami out.
