If your doctor gave you a list of do's and don'ts which has no mention whatever of sex, would you specifically ask if sex was permitted, or would you use your own judgement and decide?

If it's not a don't, it's permitted. Go ahead! :-)

If I am unclear on ANY of the doctor's suggestions, I ask for clarification. If the doctor doesn't mention sky-diving and it's in my plans, I would ask.

Doing sex is beneficial for the immune system so do sex as much as I would feel 'up'to it.

It sex was not included on the list, I would say it's okay to do it.

I would ask. Until I got an answer I was happy with (ie relatively clear and understandable)

I've had that exact situation happen before, and I asked. I believe the doctor expected (anticipated?) that a patient might ask, because she took the questions in stride and answered them just as if I had asked if it was okay to go sailing or attend a late-night party.
I liken it to medical professionals seeing people naked: it's part of the job.
As for looking at it from the patients' perspective, some people are hesitant to initiate that kind of discussion. Not in my case, though.

Couple things..  1) I would NEVER ask my doctor if I could have sex..  2)  If sex killed me, that'd be just fine.

You are right about doctors taking such queries in their stride. 

If the doctor never said you can't have sex, then you will be fine....might be good for you...
